Appointments
To schedule an appointment please call 608-339-6613. If you have a scheduled appointment and need to cancel or reschedule please provide us with at least 24 hours notice. Please arrive approximately 10 minutes prior to your first visit to fill out patient forms. Thank you.
Patient Records
Please contact your previous dentist prior to your appointment and have your records transferred to Adams Family Dentistry. Digital records may be sent to [email protected]
Insurance and Billing
We are PPO providers for Cigna, Delta Dental, Ameritas and most BCBS. We accept most commercial insurances. As a courtesy we will submit claims for you to your insurance company. Co-pays and deductibles are due at time of service.
Payment Options
We accept checks, cash, CareCredit, Master Card and Visa. Payment is required in full at time of service.
