Appointments

To schedule an appointment please call 608-339-6613. If you have a scheduled appointment and need to cancel or reschedule please provide us with at least 24 hours notice. 

Insurance and Billing

We are PPO providers for Cigna, Delta Dental, Ameritas and most BCBS. As a courtesy we will submit claims for you to your insurance company. Co-pays and deductibles are due at time of service. If your insurance company does not pay within thirty (30) days after submission, we ask that you pay the balance and that you contact the insurance company regarding settlement. We will help will the collection of the claim, but the balance is ultimately your responsibility. 

Payment Options

We accept checks, cash, CareCredit, Master Card and Visa. Payment is required in full at time of service unless prior arrangements have been agreed upon.
